Terms & Cancellation Policy

These Terms of Service govern all appointments and services provided by Nurse Shara Amzallag, MSN, NP-BC ("Nurse Shara," "we," or "us"). By submitting an appointment request or booking a service, you agree to these terms.

1. Services Provided

Nurse Shara provides in-home medical ear piercing using the Blomdahl medical piercing system. Services are available for infants, children, and adults across Los Angeles County. All services are performed by Shara Amzallag, MSN, NP-BC, a California-licensed board-certified nurse practitioner (NPI: 1639987084).

Services include:

All-inclusive pricing covers: the in-home visit, single-use sterile instruments, Blomdahl medical-grade earrings, written aftercare instructions, aftercare wipes, and a personalized piercing certificate. See the pricing section on our homepage for current rates.

2. Booking Process

Appointments are requested through the booking form on this website. Your request is not confirmed until you receive a personal response from Nurse Shara via phone, text, or email. We aim to respond to all requests within 24 hours (Monday–Friday; Sunday requests may be confirmed the following business day).

A confirmed appointment constitutes an agreement to these terms.

3. Cancellation & Rescheduling

Please provide at least 48 hours' notice to cancel or reschedule your appointment at no charge.

5. Travel Fees

Appointments within the San Fernando Valley (including Encino, Sherman Oaks, Tarzana, Woodland Hills, Calabasas, and surrounding neighborhoods) are included in standard pricing.

Appointments outside the San Fernando Valley but within Los Angeles County may incur a travel fee. The applicable fee will be communicated and agreed upon during the booking confirmation process.

6. Parental Consent

A parent or legal guardian must be present for all appointments involving a minor. By submitting a booking request on behalf of a minor, you confirm that you are the parent or legal guardian and consent to the procedure being performed.

Nurse Shara reserves the right to decline to perform a service if, in her professional judgment, the procedure is not appropriate for the child at that time — including due to recent illness, health concerns, or insufficient parental consent documentation.

7. Health & Safety Requirements

For infant appointments, Nurse Shara recommends waiting until after your baby's first round of vaccinations (typically around 2 months of age). During your consultation, she will review the child's age, health history, and readiness.

Please disclose any known allergies (particularly to metals or adhesives), bleeding disorders, keloid history, skin conditions, or immunocompromising conditions when booking. Withholding material health information relevant to the procedure may affect the outcome of the service.

If Nurse Shara determines on arrival that a service cannot safely proceed, a partial visit fee may apply.

8. Medical Disclaimer

Nurse Shara is a licensed nurse practitioner who performs ear piercing using medical-grade instruments and techniques. This service is not a substitute for medical treatment or advice from a physician. If you experience signs of infection, allergic reaction, or any other concerning symptoms following your piercing, contact a healthcare provider promptly.

Ear piercing, like any skin procedure, carries inherent risks including infection, allergic reaction, keloid formation, and improper healing. These risks are minimized through the Blomdahl system and Nurse Shara's clinical technique, but cannot be entirely eliminated.

9. Limitation of Liability

To the fullest extent permitted by California law, Nurse Shara's liability for any claim arising from services rendered is limited to the amount paid for the specific appointment in question. We are not liable for indirect, incidental, or consequential damages.

10. Governing Law

These terms are governed by the laws of the State of California. Any disputes arising under these terms shall be resolved in the courts of Los Angeles County, California.

11. Changes to These Terms

We may update these Terms from time to time. The current version will always be available at this URL. Continued use of our services after any update constitutes acceptance of the revised terms.
